Uncontrolled arrhythmias
Severe hypertension (>180/110 mmHg)
Aortic stenosis
HOCM (hypertrophic obstructive cardiomyopathy)

MI and death
Notes:
Severe hypertension and aortic stenosis significantly increase the risk of adverse outcomes during stress testing, so strict pre-procedural BP control is vital.
Prior to testing, patients with suspected arrhythmias or ischemia may benefit from pharmacologic alternatives (e.g., dobutamine for stress induction) when physical stress is not feasible.
