Contraindications

  • MI/LBBB (new)

  • Unstable angina

  • Uncontrolled arrhythmias

  • Severe hypertension (>180/110 mmHg)

  • Aortic stenosis

  • HOCM (hypertrophic obstructive cardiomyopathy)


Risks

  • MI and death


Notes:

  • Severe hypertension and aortic stenosis significantly increase the risk of adverse outcomes during stress testing, so strict pre-procedural BP control is vital.

  • Prior to testing, patients with suspected arrhythmias or ischemia may benefit from pharmacologic alternatives (e.g., dobutamine for stress induction) when physical stress is not feasible​​​.
