SSSNAPW Recommendations

Definition: Lifestyle recommendations promoting well-being and managing chronic conditions (e.g., CVD, diabetes, obesity) through stress, sleep, smoking, nutrition, alcohol, physical activity, and weight management.

Aetiology/Causes:Poor lifestyle factors, including stress, poor sleep, smoking, poor diet, excessive alcohol, inactivity, contribute to chronic conditions like cardiovascular disease, hypertension, diabetes, and obesity.

Pathophysiology:

  • Stress: Raises cortisol, increasing risks of hypertension, obesity, CVD.

  • Sleep Deprivation: Disrupts metabolism, exacerbates conditions like diabetes, heart disease.

  • Smoking: Accelerates atherosclerosis, raising risks for respiratory and cardiovascular diseases.

  • Nutrition: High saturated fat, refined sugar, salt intake increase risks of obesity, hypertension, heart disease.

  • Alcohol: Excessive use damages liver, increases CVD risk.

  • Physical Inactivity: Leads to weight gain, poor cardiovascular fitness, increased diabetes, and hypertension risks.

Symptoms:

  • Stress: Anxiety, irritability, concentration issues, muscle tension.

  • Sleep Issues: Fatigue, irritability, poor concentration.

  • Smoking Effects: Cough, breathlessness, infection susceptibility.

  • Poor Nutrition: Weight gain, high cholesterol, high BP.

  • Alcohol Overuse: Liver damage, high BP.

  • Inactivity: Weakness, fatigue.

Investigations:

  • Stress: Perceived Stress Scale, referral for CBT.

  • Sleep: Sleep studies for apnea; sleep hygiene assessment.

  • Smoking: Spirometry, smoking history.

  • Nutrition/Obesity: Diet assessment, weight, waist circumference, cholesterol, glucose.

  • Alcohol: AUDIT-C, liver function tests, BP check.

  • Physical Activity: Fitness level, strength, and cardiovascular assessments.

Management:

  • Stress: CBT, smoking cessation, sleep hygiene.

  • Nutrition: Balanced diet, 5 serves of vegetables, 2 of fruit; reduce salt (<5g), limit sugary drinks, red/processed meats; dietitian referral if needed.

  • Alcohol: ≤2 drinks/day, max 4 on one occasion; for HTN, <1 drink/day.

  • Physical Activity: 30 mins moderate exercise/day; muscle strengthening 2 days/week; reduce sitting.

Complications:

  • Stress: Mental health issues, CVD, diabetes.

  • Poor Sleep: Obesity, CVD, accidents.

  • Smoking: Respiratory/CVD, cancers.

  • Nutrition Issues: Obesity, diabetes, heart disease.

  • Alcohol Overuse: Liver damage, hypertension.

  • Inactivity: Muscle weakness, CVD, metabolic disorders.

Prognosis:Improved health outcomes with stress management, sleep improvement, quitting smoking, healthy diet, regular physical activity.

Notes:

  • Salt Intake: <6g/day (<2g sodium), especially for heart failure, CKD, HTN.
