Pitted Keratolysis



Definition

  • Superficial bacterial infection of the soles, causing whitish, malodorous skin with "punched-out" pits


Aetiology

  • Bacteria: Corynebacteria, Dermatophilus, Kytococcus sedentarius

  • Moist environments promote bacterial growth → stratum corneum breakdown → sulfur compounds cause malodour


Risk Factors

  • Occupations with prolonged moisture exposure (farmers, athletes, sailors)

  • Hot, humid climates, hyperhidrosis, occlusive footwear

  • More common in males


Symptoms

  • Whitish, pitted skin on soles

  • Foul odour (most prominent feature)

  • Pits may merge into larger lesions

  • Can be asymptomatic or cause discomfort


Management

  • Topical clindamycin 1% lotion BD x 10 days

  • Manage hyperhidrosis (antiperspirants, foot hygiene)
