Necrobiosis Lipoidica

  


Definition

  • Rare granulomatous skin disorder affecting shins, mainly in diabetics (Type 1 & 2) but can occur in non-diabetics

  • Exact cause unknown


Clinical Features

  • Location: Shins (bilateral), rarely elsewhere

  • Appearance: Yellow-brown patches with central atrophy, telangiectasia, shiny surface

  • Shape: Round, oval, or irregular

  • Symptoms: Usually asymptomatic, may be tender

  • Complications: Ulceration after minor trauma, risk of secondary infection


Differential Diagnosis

  • Elastolytic giant cell granuloma

  • Necrobiotic xanthogranuloma

  • Sarcoidosis


Diagnosis

  • Clinical appearance

  • Biopsy (if needed): Granulomatous inflammation around necrobiotic collagen (distinguish from granuloma annulare, rheumatoid nodules)


Management

  • Treatment not always required

  • Options:

    • Topical steroids (under occlusion)

    • Intralesional steroids or systemic corticosteroids

    • Treat secondary infections if ulcers develop
