Definition
Serious skin cancer due to uncontrolled melanocyte growth
Melanoma Rules:
"Ugly duckling" sign: A lesion that looks different from others
Changing pigmented lesion → suspicious
Elevated, firm, or growing → consider biopsy
Types
Superficial Spreading Melanoma (Most common)
Younger patients, sun-related
Irregular pigmentation, horizontal growth
Nodular Melanoma
Vertical growth, firm, raised
Not always sun-related
Lentigo Maligna Melanoma
Elderly, sun-damaged skin
Common on the face
Acral Lentiginous Melanoma
More common in darker skin
Palms, soles, mucous membranes, worse prognosis
Diagnosis (ABCDE-EFG Criteria)
Asymmetry – One half differs from the other
Border – Irregular or poorly defined edges
Colour – Varies within lesion, may have hypopigmented areas
Diameter – Typically >6 mm, but smaller lesions can be early melanoma
Evolution – Changes in size, shape, or colour; may bleed or itch
Elevation – Raised and palpable
Firmness – Malignant lesions are firm; benign are soft
Growth – Increasing in size is concerning
Management
Initial excision with 2 mm margin
Wide excision if confirmed melanoma
Breslow thickness = most critical prognostic factor
If >1 mm thick or >0.75 mm with high-risk features (ulceration, high mitotic rate) → Refer for sentinel lymph node evaluation
