Flushing



Definition

  • Blush: Temporary facial redness

  • Flush: More intense redness with warmth, sweating, and goosebumps

  • Common sites: Face, neck, upper chest, ears

Aetiology & Causes

Common:

  • Menopause, rosacea

  • Emotional stress, temperature changes

  • Hot/spicy foods, alcohol, exercise

  • Drugs: CCBs, sildenafil, corticosteroids

Less Common:

  • Carcinoid syndrome, phaeochromocytoma, neurological disorders

  • Systemic mastocytosis

  • Food additives: MSG, nitrates, nitrites

Management

  • Treat underlying cause

  • Review & discontinue triggering medications


Menopause-related flushing:

  • Short-term HRT if appropriate


General flushing:

  • First-line: Propranolol 10 mg BD or PRN

  • If no response: Clonidine 25–50 mcg BD

  • Dermatology referral for refractory cases (e.g. carvedilol use)


Response time:

  • Episodic condition; assess treatment effectiveness after 4–6 weeks
