Pilar Cyst (Trichilemmal Cyst)

Definition

  • Keratin-filled cyst from the outer root sheath of hair follicles

  • Common on the scalp, firm, mobile, lacks a central punctum

  • Often misdiagnosed as epidermoid or sebaceous cysts

Epidemiology

  • Middle-aged females most affected

  • Autosomal dominant inheritance (50% chance of passing to offspring)

Symptoms

  • Usually asymptomatic, but can cause discomfort if inflamed or infected.

Management

  • Removal is not necessary unless symptomatic. If required, surgical excision under local anesthesia is preferred before rupture, which could lead to scarring.
